As a former healthcare administrator, Brett founded Chosen Family Home Care to create a more personal impact on care. He’s passionate about serving Philadelphia’s seniors and LGBTQ+ community through inclusive, family-focused support. Brett stays involved in daily operations and is committed to matching clients with the right caregiver while fostering connection, communication, and dignity in every plan.

Cianni Vera

Front Desk Coordinator/Client Care Specialist

Hi! My name is Cianni Marie Vera. I’m currently a Front Desk Coordinator/Client Care Specialist at Chosen Family Home Care, where I enjoy helping people and keeping things running smoothly. I’m the oldest of five siblings, so I’ve always had a natural love for caring for others—which is a big part of why I’m in school right now to become an elementary school teacher. At home, I have a fun little crew of pets: one cat named Kali and four dogs—Rex, Minnie, Zeke, and Zeus.

In my free time, I love spending time with my friends and family, and I’m always up for anything crafty. Whether it’s making something by hand or just getting creative, that’s definitely my happy place.

Yemarie Mendez

Client Care Coordinator

I, Yemarie have over eight years of experience in the Home Care industry. As a Client Care Coordinator I bring compassion and genuine love for helping others in Chosen Family Home Care. I'm known for my strong communication skills and consistently delivers outstanding service and support to those in her care. My warm personality and commitment to make people smile creates a comforting and joyful environment. I take pride in creating meaningful connections with clients, caregivers and her peers. Outside of work, I enjoy sharing good food, quality time and new adventures with my family, who remain the heart of my inspiration.

Jonathan Colina

HR Manager

Jonathan Colina is originally from Venezuela. He moved to the United States and later went to Penn State University, where he studied and prepared for his career. After college, Jonathan worked as an HR specialist and recruiter for several companies, gaining over 10 years of experience in the field. In January 2023, he joined Chosen Family Home Care. He has been with the company for about 2 and a half years and has helped it grow. Jonathan enjoys being a team player and always works hard to support his coworkers and the company.

Partnerships & Operations Liaison

Hi there! I am the Operations and Partnerships Liaison for Chosen Family Home Care and CareBridge Academy, where I bridge compassionate care with career-building education. I am fully bilingual, I started my own journey as a caregiver too!

Tim Deserable

Director of Operations

Tim Deserable, MS, is the Director of Operations at Chosen Family Home Care, where he leads agency operations, caregiver support, and client services. With a strong background in home care leadership, Tim is dedicated to ensuring families receive compassionate, reliable, and high quality in-home care across Philadelphia and surrounding counties.

Melody Naroleski

Office Manager

I have spent most of my career in customer service, restaurants, retail, and orthopedic medicine. I always appreciate a job that constantly challenges me. I'm passionate about helping people and creating positive experiences in every interaction. Outside of work, I'm a proud cat mama,and a devoted fan of all things 80s. My greatest joy is my son, who is truly the light of my life. I'm also a die-hard Philly sports fan—Go Phillies and Go Birds! that's me:)
